Write an Application to the Lottery Authority
Write an application to the lottery authority with the title “Application for the Lottery Prize Claim” to the Directorate of State Lotteries, Government of Nagaland.
There are two offices to claim the Nagaland Lotteries Prizes. The first one is

And the Second is
The Nodal Officer, Kolkata Camp, Directorate of Nagaland State Lotteries,
Nagaland House, 1st Floor, LA-Block, Plot No-6, Salt Lake, Sector 3, Kolkata – 700 098
You have to choose one out of them which is near to your Home or the office.
Then, Write your Full Name and Permanent Address. Write your Father or Husband’s Name. Tell the Profession of the Father or Husband’s. Write down the Ticket Number for which you have Won the Lottery Prize. Tell the Draw time for which you have won the Lottery Prize. As above you play in the 6:00 PM draw. So write it in the application. Then tell the Lottery Name and the Date on which you play the lottery game.
Moreover, Provide your PAN (Personal Account Number) that issued by the Income Tax Department of India. Provide your Contact Number and Aadhaar Card Number. Tell the Prize amount which you have won in the lottery game.
Provide your four passport-sized pictures with the application. Give your bank account number. Provide your IFSC Code (Indian Financial System Code) to transfer the prize amount to your bank account. IFSC code is given on the chequebook that provided by the relevant bank.
Write the application with complete attention after that no changes can be made to claim the prize amount.
In Addition, The Application should be filled properly with the Name of the ticket holder, Postal Address, Time of the Lottery Draw, Amount of the Prize, PAN Code, Aadhaar Card Number, Contact Number, and Bank details like Bank Account Number, IFSC Code, Name of the Bank and Branch, cheque, etc. and the number of claims of prizes in the Current Financial Year.
Again, Original Prize-Winning ticket with one photocopy properly attested by an Officer / Notary Public, along with Four Passport-sized photographs of the ticket holder, an affidavit of ownership of the prize-winning ticket from any 1st Class Magistrate/Notary Public should also attached.
The government of Nagaland shall not be responsible for paying prizes on tickets lost by the postal staff, defaced, laminated, or cut up.
Further, Color photocopied, tampered, Forged, fabricated, duplicated, altered, printed anywhere apart from the assigned security printers, purchased from any broker, middleman, or agent, obtained from the original winner in an illegal way or any other method, or found to be illegal in any form shall not be considered to get any prize and any person claiming such prize money is responsible to be sued in a court. Such Ticket Holder shall be responsible for appearing before the Director, Nagaland State Lotteries, or the Appropriate Authority as and when called for the Inquiry.
There will be an amount of income tax on the claimed prize of the lottery game. You have to provide the information of the Seller or the Sub Stockist from where you have purchased the lottery ticket.
Then, You have to provide a ₹ 20 Stamp Paper for the Declaration. Take oath on the Stamp Paper that you will not provide any forged or false information in the claim application. If you provide such forged or false information you have to face legal proceedings from the Lottery Administration and that will not be good for you when you win another prize in the lottery game.
